I understand by submission of this form, it will place my child on the Wait List, but does not guarantee care available by the date of care required (as noted above). I undersand this is a non-refundable processing fee, and will not be applied to my child's tuition.

I understand by enrolling in Before / After Care - I am responsible for contacting my child's school district/transportation department to request The Academy as a drop off and pick-up site. I will contact The Academy with my child's bus number.
